    <description> &lt;p&gt;Ahh, its okay guys, I am just going to get them to download and print it. There is some sensitive information on it that I need to secure, which means more headaches to get it to work properly.&lt;/p&gt;

    <description> &lt;p&gt;Yes, that is fine, I dont care if they dont see the form online when filling it out, it can be just fields on a blank page, I just want it to print those fields on the form in correct order on the dotted lines when they click print. Is that what you mean in your &quot;best method&quot;?&lt;/p&gt;

    <description> &lt;p&gt;I would say there are a couple methods you could go about to do this.  First of all, starting from scratch and coding it out to resemble this page.  Second, you could scan a copy of this page and then code a page using absolute positioning and that kind of CSS deal to place the text over the lines.&lt;/p&gt;
&lt;p&gt;Either way, I would say your best method would be to simply have a form that they fill out, and then when they are finished and click &quot;Submit,&quot; a form that looks like this (but filled out with their information) is displayed, and they can print this out to do whatever else they need to do.&lt;/p&gt;

&lt;p&gt;Just need them to type on the dotted line, and when they print it, I want it to print exactly where they typed it (on the dotted line.) Then they can sign it and fax it off.&lt;/p&gt;
&lt;p&gt;I think this is what stylized is correct? How could I make it the form I have as the style?&lt;/p&gt;

&lt;p&gt;Sure,&lt;/p&gt;
&lt;p&gt;Just use a normal html form with a javascript print page button at the bottom.&lt;/p&gt;
&lt;p&gt;You could use a series of text boxes, text area boxes, radio buttons, and drop down select boxes.&lt;br /&gt;
Think it as creating an email form but without the submit button. &lt;/p&gt;
