The answer your looking for is rather complex, but very possible.
You will have to code the pages yourselfs... in PHP.

Or if you want a much simpler, easier to build form, then use the method I used... You should be wanted, dont make the same mistake I did, The REg 1 &2 need merging, and same for Login 2 & 2.
