    <description> &lt;p&gt;If I knew where amazon&#039;s form was sent (email address, etc.) then i wouldn&#039;t have a problem, I could make my own form and forward it to the same address. I don&#039;t know if it is sent to an address. The main goal is that I don&#039;t want my customer to be inconvenienced by getting sent to another company&#039;s page/I don&#039;t want to lose my customer to another company.&lt;/p&gt;
