    <description> &lt;p&gt;I was looking into this awhile back. I found two that were half decent, but they both neeeded a lot of work to get them to be standards compliant. I have temporarily abandoned using software at all now in favour of designing my own php forms. The options I found were just too poorly designed.&lt;/p&gt;
&lt;p&gt;The ones I tried are:&lt;/p&gt;

&lt;p&gt;There&#039;s another one I found that was just called &quot;Open Source Survey&quot;. I tried the online demo and found it hard to use so I didn&#039;t go with that one. &lt;p&gt;Let me know if you find anything good - I still haven&#039;t given up on using a pre-made solution for this.&lt;/p&gt;
