I was looking into this awhile back. I found two that were half decent, but they both neeeded a lot of work to get them to be standards compliant. I have temporarily abandoned using software at all now in favour of designing my own php forms. The options I found were just too poorly designed.

The ones I tried are:

PHP Surveyor - free, open source, but the templates were all table based which was obviously not acceptable to me. The admin control panel is terrible too.

Simple Web Survey - I liked this one but the project doesn't seem to have been updated for a few years so that put me off.

There's another one I found that was just called "Open Source Survey". I tried the online demo and found it hard to use so I didn't go with that one. On a hotscript search a couple of other options come up that I haven't looked at yet.

Let me know if you find anything good - I still haven't given up on using a pre-made solution for this.
