i want one that uses a mysql database but i cant find a good one and i cant make it so can someone give me one say someone has made one for themself maybe they could give me a copy of it or something like that

You can do this with a script that edits the htpasswd file directly, eliminating the need for a database. If you get over about 50 logins, go for the database option, or a simple text-files script like http://www.designanet.co.uk/phpAuth/index.php
