    <description> &lt;p&gt;Since you&#039;re using Javascript to open up the &quot;add more fields&quot; category, and it doesn&#039;t...i would say one of two things.  Either the path to the JS file is wrong, or coded incorrectly (possibly got corrupted?).&lt;/p&gt;
