    <description> &lt;p&gt;Make sure the form is valid (start and end form tags etc). are you using any CSS with the form and are you using a DOC tag?&lt;/p&gt;
&lt;p&gt;Tried using require instead of include?&lt;br /&gt;
How about just having variables in the page and using an if statement or something.&lt;/p&gt;
