personally, i'd stay away from any host that has a set up fee, like hostrocket, there's too much competition in the host market for set up fees.

HostRocket.com comes to mind - they offer all those features for $10-$15/month. I use them with no problems.

As has been mentioned, there are PLENTY of hosts who could meet your requirements. There are also quite a few hosting directories that contain the names/web sites of various hosts, such as the one at hostsearch.com . Rather than giving you advise as to which host to choose, here are some general tips:
- The first and most important step in selecting a host is RESEARCH. Check message boards like this one, and the one at webhostingtalk.com, as well as the review pages on hostsearch.com . This will help prevent you from choosing a bad host.

- Email the host's support department and make sure they know what they are talking about. You might want to try a letter such as the one here: http://www.hosthelp.com/questions.php

- Be sure to get a feel for the character of the company you're thinking of signing up with. It's easy for them to seem great when everything is functioning well, but how will they respond in the case of an emergency? If your site is down, will they argue about it or will they immediately issue a credit? Be sure that your web host is a company that you can trust.

- Avoid hosts that lie by using tactics such as "unlimited bandwidth"

I am kind of partial to Hostmatters. There are a ton of hosts out there that would fit your plans, though.
