it is almost that what i mean but instead of "somescript.pl" i'm looking for a poll that can handle 11 questions.

I don't understand what you mean by "hidden questions". I guess you mean a form, that has 11 questions, with 11 dropdown menus.

You can use multiple inputs within the same form. They just need different names, so perl will know the name-value pairs. As an example here is a simple form that has multiple questions and multiple dropdown menus.
