&lt;p&gt;Is your server Unix or NT? If it&#039;s NT you can use ASP and just use request.form(&amp;quot;field name&amp;quot;) to get the values from the bonus form and then format it and send it to yourself using CDONTS.&lt;/p&gt;
&lt;p&gt;If you&#039;re using a Unix server you should probably use a CGI script to do this. I&#039;m not a CGI expert so I can&#039;t really comment on this but I know there are TONS of examples out there.&lt;/p&gt;
&lt;p&gt;The third option would be using javascript and have the form call itself. This would be a little tricky since it&#039;s easier to create a mail session on the server, but it is possible.&lt;/p&gt;
&lt;p&gt;If you would like an example in ASP let me know.&lt;/p&gt;
