I like the look of the site a lot. Nice and clean and simple.
One think that would be helpful though is if you put hieght and width attributes in your image tag. I keep thinking that I'm missing something while staring at the form until the picture pops up a couple of seconds after.

Yeah, its good. DO something about the size of the images however, a little reduction would aid download speed for dial up users immensley
