New site feedback forms

Am about to set live a new purchase engine on my site - am thinking about having a pop under window for feedback, just want to see what the customers think of the new forms etc.

Has anyone got any sage advice here?
Is there a preference between having 1 page with 5 questions, or 5 pages with one question each - and what are the drawbacks of these ie are people much less likely to fill in one of them?

What kind of questions are they? If they are long response ones (textareas), I would do a few on each page. If they are check boxes then I would do all on one page.

I would recommend you just have them check boxes, not a lot of people will want to fill in a text box.
