It looks like it is trying to post to a page that is not there. The formail script should be in http://ink-price1.com/cgi-bin/formmail.pl .

You just have it as action="cgi-bin/formmail.pl" adding the http: bit before it can sometimes make all the difference. Otherwise is this on the same host/server as the ones you have used it on before?

It looks like you might have formmail setup incorrectly... When called from the browser, this is what FormMail SHOULD look like:
http://www.intersurge.com/cgi-sys/formmail.pl

Try re-installing formmail (make sure you upload in ASCII mode and chmod correctly) and see if that resolves the problem.

Reinstalled it twice... ASCII mode upload....tried CHMOD 755 and 777. I guess I will just try redesigning the page...line by line...what a pain.

Well, as I mentioned, it looks like a problem with how the script itself is setup... It's displaying the results page when called from a web browser -- something that it definitely should not do. Can you test it on another server and see if it works?
