I want to create a form that includes the following, please help if you can....
Name
Company:
upload: (there will be a browse button to select files to upload)
submit button.
after clicking on the submit button, it will upload the file to a destination and an notify email will be sent.
Anybody know how to do that?

I posted this same question a few days ago. Somebody replied and directed me to one of their previous posts that has a script that will do this but I couldn't get it to work:

I think you should do it with a server side script. The uploading would require php/asp/perl. It shouldn't be too hard but you might have to learn one of those languages!

depends if you want it done in ASP or PHP or ASP.NET
