I'm looking for a very basic forum script that could easily be implemented into an existing site and use the username and password from an existing database.

I don't think you'll be able to find one that'll support username/passwords from an existing DB off the bat, however hacking a phpbb install shouldn't be too difficult.
The industry standard and software in use here is vBulletin, it's very good but you do have to pay for a license.
