Hi, Consultant needs form...
Hi, I'm looking for a consultants questionaire for developing a new website. My customer has a "committe" to decide how their new website wants to look so I want to start out with some order to keep things from getting hairy. I also want to have them communicate with me with forms so I dont end up running around like a chicken with my head cut off! Any suggestions?

Sounds like you may need a database to store all ths information as well, check out hotscripts.com for forms and/or database option, or if you wanted to run like a dummy site you could use something like phpnuke and use the forums and polls etc (is made up of modules, just use the bits you need)
All of the above will need to be run with PHP or similar

I believe Sitepoint.com has a lot of stuff freelancing and I know they have some preset forms available. There is a package you can buy there but they might have some stuff freely available.
