Not in a hospital (seen this with family and isn't nice)
When I go, I don't want to be asleep, or on an operating table, or just smiling nicely to a loved one as I pass away.
I want to be shouting "Wooooooohaa" just before I enter the next stage
Something like doing a wheelie at 180mph on an MV Agusta F4 1000, you know, going out full of adrenaline in a blaze of flames

My mother is 75, she was one of the youngest Abstract Expressionist artists in NYC in the 50s and 60s and now pretty much they are all dead except her.
Recently, her best friend was killed by her own son in a murder-suicide (not a pleasant way to go)
In the Jewish tradition, it is a mitzvah to die while reciting the Shema
Got loads of time now to do all the things I hadn't got time to do when working full-time.
Hoping I've got at least another 30 years. Half way through writing my first novel. Now have time to do websites, learn coding, create computer games, and the hundred and one other exciting things there are to do in this day and age (I was born too soon. Should have been born in the computer age. Love it).
Not scared of dying. But scared stiff of dying before I've had time to finish all the things I want to do with my extra time.
By the time I'm in my 90's I hope to have finished. When I've finished, then I'd like to just die peacefully in my sleep. Preferably after my wife. wouldn't want her waking up in the morning finding me dead.

Actually i did not like to die. But death is a real fact. So i wish to die very calmly that means without suffering any pain.
