I just ran CCleaner (formerly "Crap Cleaner")
It removed about 4 gigs of temporary files, file fragments, and junk
I also tweaked my registry - I knew there were a lot of bad links there
Now the performance improvement is impressive!
[note] be sure to create a restore point and back up your registry before you go deleting stuff! I did not experience any problems, but you never know!

Yup, I use it all the time, too. It also gets rid of startup errors some computers get when a program doesn't uninstall correctly.

The cleaning also fixed a problem I was having with Mozilla apps (Firefox and Thunderbird) becoming unresponsive, and requiring a visit to Task Manager to kill them and restart. YAY!

However, I am still having a problem with Windows Explorer crapping out periodically (in Vista Home Premium) and the only fix I found online was one I already use, Task Mgr to kill it and restart...seems to be fixed
